Right to Inspect and Copy
You have the right to inspect and obtain a copy of your PHI in our designated record set. To request access, submit a written request to us. We may charge a reasonable fee for copying and postage. We will respond within 30 days.
Right to Request Amendment
If you believe your PHI is incorrect or incomplete, you have the right to request that we amend it. Submit your request in writing, including the reason for the request. We may deny your request in certain circumstances.
Right to an Accounting of Disclosures
You have the right to request an accounting of certain disclosures of your PHI made in the six years prior to your request. Your request must specify the time period. The first accounting in any 12-month period is free; we may charge a reasonable fee for additional requests.
Right to Request Restrictions
You have the right to request restrictions on certain uses and disclosures of your PHI for treatment, payment, or healthcare operations. We are not required to agree to your request, except that we must agree to a restriction on disclosure of PHI to your health plan if the disclosure is for payment or healthcare operations and the PHI relates solely to a service you have paid for in full out-of-pocket.
